No approved method of hand drying was observed at the handwashing sink where the automatic dish machine is located, as the machine was stuck and not providing paper towels. A handwashing sink was blocked or inaccessible next to the three-compartment sink in the back of the facility, due to an extended pole and bottled beverages. No handwashing cleanser was available at the handwashing sink on the main kitchen line. The mop sink was not maintained in good repair, unable to drain grey water and had standing water. No quaternary ammonium compound solution was utilized to adequately achieve sanitization in the kitchen. Ready-to-eat time and temperature-controlled for safety food (sliced meats, prepared vegetables, shredded cheeses) in reach-in refrigerators had no date marking indicators. Some products had previous date markings with no information filled out.

Rising Star Chinese Eatery, 7001 San Antonio NE

The facility lacked designated areas for employee dressing, personal belongings. An employee washed their hands in the three-compartment sink instead of a designated handwashing sink. Employees were observed not washing their hands when required, specifically when returning from outside and touching their body and hair. A handwashing sink near the main cook line was blocked and inaccessible by a silverware rack. The ventilation hood system had heavy grease buildup and inadequate collection of grease and condensation. Lighting was inadequate on the main cook line due to a burned-out light bulb, and light bulbs in this area lacked protective shielding. Clean equipment was stored improperly, not allowing for self-draining or air drying. Nonfood contact surfaces on and under the wok station had substantial grease buildup. Floors and walls in the kitchen had significant food and soil debris buildup, especially under the wok station. A soiled mop was found stored directly on the floor when not in use. There were inadequate air gaps at floor drains under the wok station and in the servers’ station. Raw meat was stored directly over vegetables in a prep top cooler, leading to potential cross-contamination. Uncovered food items, including tempura chicken, cut mushrooms and cut zucchini, were found in reach-in coolers. The person in charge could not demonstrate knowledge of foodborne disease prevention or Food Code requirements. Food employees lacked food handler cards or proof of approved food safety training. The establishment did not have a certified food protection manager on staff. Temperature logs for TCS foods were not available, nor were calibration logs for temperature measuring devices. Wiping cloths were used improperly for covering egg rolls and stored on tables and cutting boards. TCS food items (diced peppers, chopped vegetable mix, chopped broccoli) in a cold holding unit measured above 41 degrees. The ice machine in the server’s station had black and brown organic substance buildup on its food-contact surface. Ready-to-eat TCS food items lacked date markings in coolers throughout the kitchen and the server’s station.

Sharetea, 6125 Montgomery NE

Febreze air freshener and Windex glass cleaner were stored improperly near single-service articles, posing a contamination risk. An employee did not wash hands when required, specifically when switching tasks like operating the cash register and preparing drinks. The handwashing sink near the coffee machines was blocked by a coffee filter basket. No approved method for hand drying was available at the handwashing station near the coffee machine. An employee was observed wearing bracelets on both wrists while preparing drinks. There was debris buildup on the floors under the counter and prep top cooler. Inadequate air gaps were observed at floor drains under the coffee make station, near the three-compartment sink and near the storage shelf. Floor drains under the coffee make station, near the three-compartment sink and near the storage shelf were not properly maintained, with significant debris and missing strainers. No testing kit or device was available to measure the concentration of chemical sanitizing solution. The ice machine had an orange, slimy substance buildup in direct contact with stored ice. The drop-in freezer had extreme syrup and debris buildup along its seal and interior.

Smith’s Deli, 6125 4th NW (Class 5)

Unlabeled spray bottles containing unknown materials were observed. Poisonous or toxic materials, including an unlabeled spray bottle, food-grade lubricant, and non-food-safe Goo Gone, were improperly stored near a handwashing sink and cashier counter, posing a contamination risk to single-service articles. A handwashing sink was misused by storing a quat sanitizer spray bottle on it. TCS food, specifically fried chicken thighs, breast and tenders, was not hot held at or above 135 degrees. Nonfood contact surfaces under and around food display units had extreme food debris and soil buildup. An inadequate air gap was observed at the Deli station. Food was not protected from contamination as the meat slicer was not cleaned and sanitized between uses.

Wings Gone Wild, 101 98th NW (Class C)

No handwashing cleanser was available at the handwashing sink in the public/customer restroom. Clean utensils were improperly stored, not allowing for self-draining or air drying. Freezer door gaskets had substantial soil and food buildup. The handwashing sink in the kitchen was leaking. The three-compartment sink faucet in the kitchen was leaking. Single-service articles were unclean, stored face-up, and unprotected. The person in charge could not provide employee food handler cards or training records. The establishment did not have a certified food protection manager on staff. TCS foods in coolers were cold-held above 41 degrees, including chopped green chile, unmarked meat, raw bacon, chopped garlic, pico de gallo, jalapeno sauce, raw shelled eggs and unmarked dressings/sauces. Ready-to-eat TCS foods lacked date markings in upright and undercounter coolers.
